PHARMACIST - STAFF (PER DIEM - VARIABLE 10 Hr. Shi

• Work directly with the medical staff to provide medication therapy assessment, consultation and management
• Manage medication therapies via medical staff approved collaborative practice agreements. These include: anemia management protocol, anticoagulation therapies, antimicrobial stewardship program, pharmacokinetic analysis, parenteral nutrition, etc.
• Clinical pharmacy activities are performed by staff throughout the hospital in our decentralized patient care area locations
• Daily team rounding
• Specialty pharmacy services include: Emergency Room Pharmacist, Oncology Pharmacist, Pediatrics Pharmacist, Critical Pharmacist, Operating Room Pharmacist, Telemetry Unit Pharmacist
• Participation in clinical quality committees, medication safety committee and medical staff committees
• 10-hour shifts, rotating weekends.

EDUCATION:  Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE).

REQUIREMENTS:   Current California Pharmacist License in good standing with the California Board of Pharmacy.  Two years of hospital experience preferred. Hospital pharmacy residency preferred.  Must possess a specialized pharmaceutical knowledge in addition to a good working knowledge of pharmacokinetic dosing and nutritional support.  Effective verbal and written communication skills. Computer working knowledge.
